Amy McMillin
PHYSICAL THERAPIST & OWNER
Custom Care Rehab founder, Amy McMillin, started her career in physical therapy in 2007 in the Cleveland area. She graduated from Akron University with a Bachelor of Education and earned her Master of Science in Physical Therapy from Walsh University. Her background includes orthopedic and geriatric physical therapy, over time, however, she honed her specialty to inner ear disorders, migraine, and concussions. In 2016, she obtained her Herdman Vestibular Certification from Emory University and became an Impact Trained Physical Therapist (ITPT) in treating concussions. Also, recently passed the Johns Hopkins University vestibular competency course in 2023

In addition to patient care, she has been involved in many community and hospital-wide education speaking engagements including:
“Differential Diagnosis of Dizziness in the Concussion Patient,”
“Beyond the Epley Maneuver”
“Vestibular Dysfunction in the Elderly,”
“Vestibular and Ocular Dysfunction in Concussions” at Kent State University Athletic Training Department.
On Panel Presentation for AAOF: American Academy of Otolaryngology-Head and Neck Surgery on “Should Vestibular Rehabilitation be a Part of Comprehensive Treatment for Vestibular Schwannoma”
She previously attended the 2017 American Physical Therapy Association NEXT Conference in Boston to present her poster exhibition on the topic of “Occurrence of Multi-Canal BPPV in Severe Head Trauma from a Gun Shot Wound.” Most Recently, in 2019, she was a guest speaker for the Cleveland Chapter of the Acoustic Neuroma Association for Vestibular Rehabilitation. Amy also enjoys volunteering for Stewarts Caring Place, which offers free cancer wellness services to patients in need. She is also an active member of the Hudson Chamber of Commerce and Working Women Connection.
Meghan Fonce
DOCTOR OF PHYSICAL THERAPY
Meghan received her Doctorate in Physical Therapy from the University of Mount Union. She and her husband Rocco, their son, and their chickens reside in Medina. She primarily specializes in orthopedic conditions as well as balance and postural training. She has a specialty in manual therapy techniques, including cupping, myofascial release, deep trigger point relief, and kinesiotaping.
She enjoys working with runners and weekend warriors who are looking to stay active and pain-free. She also has a passion for working with young female athletes, as she was head volleyball coach for Valley Christian School in Youngstown.
Meghan offers Physical Therapy in Strongsville, Medina, Wadsworth, Akron, Hudson, Cuyahoga Falls, Stow, and the surrounding areas.
Maddie Lautt
Maddie and her husband, Wyatt, moved to Cleveland this past June from Fargo, North Dakota. She earned her Doctorate in Physical Therapy from the University of North Dakota. She started with the Custom Care Rehab team in February 2025 but has been a PT for 3 years. She treats a variety of patients presenting with orthopedic conditions, vestibular disorders, and TMJ issues. She focuses on manual therapy techniques including the use of deep tissue massage, cupping, instrument-assisted soft tissue mobilization, as well as kinesiotaping. Maddie is also a Certified Dry Needling Clinician.
Maddie enjoys working with individuals of all ages who have a desire to thrive in their work, recreational hobbies, and day-to-day activities. She is a yoga sculpt instructor, plays pickleball, and enjoys reading, photography, and coffee. She loves her two cats, Smoky and Finn. She is excited to build one-on-one relationships with her clients and create personal goals that they can achieve together!

Chris Smolko
PHYSICAL THERAPIST
Chris received his Bachelor of Science degree in Physical Therapy from Youngstown State University in 2002. With years of experience and education, he has advanced training in spinal mobilizations, joint manipulation, and dry needling. He specializes in treating orthopedic injuries, specifically low back pain, knee injury, neck pain, and shoulder injuries.
He enjoys treating athletes of any age to keep them in sports, enhance their performance, and improve their quality of life. His calm demeanor, patience, and empathy have patients calling to continue to work with him for all their physical needs.
Chris offers Physical Therapy in Gates Mills, Beachwood, Chagrin Falls, Solon, Aurora, Bainbridge, and the surrounding areas.
Anna Ruth Gsellman
DOCTOR OF PHYSICAL THERAPY
Anna Ruth received her Doctorate of Physical Therapy from Daemen University. She enjoys working with patients from all walks of life and her background includes pediatric, geriatric, and orthopedic physical therapy
She has a specific passion for new moms since as a mother of 3 little ones, she personally understands the physical needs and challenges that come with pregnancy and postpartum. Not only does she work with women through their pregnancy-related changes, but through menopausal years as well. Anna Ruth enjoys helping women prevent and manage osteopenia and osteoporosis, ensuring they live long and active lives.
She and her family moved from upstate New York to the farms of Medina. They enjoy working their farm, planting trees, making jam, and keeping up with their three boys!

Erin Porter
DOCTOR OF PHYSICAL THERAPY
Erin earned her Doctorate of Physical Therapy from the University of Mount St. Joseph in 2012. With over 13 years of experience in both inpatient rehabilitation and outpatient neuro settings, she has developed a strong clinical focus in treating individuals with spinal cord injuries, stroke recovery, and other complex neurological conditions.
She became certified in dry needling in 2022 and earned her Certified Neuro Specialist certification in 2024, further deepening her expertise in neurorehabilitation and evidence-based interventions. Passionate about helping individuals overcome mobility challenges, she is dedicated to empowering her patients to reclaim independence and improve their quality of life.
Erin currently lives in Northeast Ohio with her family and two dogs. She is committed to providing compassionate, individualized care that supports her clients’ physical and emotional well-being.
