Custom Care Rehab, LLC, a Northeast Ohio concierge physical therapy practice founded by physical therapist and CEO Amy McMillin, has been named one of the 2026 CO—100: America's Top 100 Small Businesses by the U.S. Chamber of Commerce and CO—.

Custom Care Rehab was selected as a 2026 Culture Champion, one of 10 categories recognizing small businesses across the country for their achievements and impact. The 2026 CO—100 honorees were selected from more than 12,000 applicants and represent businesses across 36 states and Washington, D.C.

Founded during the COVID-19 pandemic, Custom Care Rehab began as a one-person mobile physical therapy practice created to bring expert care directly to patients who could not—or did not want to—navigate the traditional clinic model.

What began with Amy McMillin seeing patients on her own has grown into a team of physical therapists and wellness professionals serving clients throughout Cuyahoga, Summit, and Stark counties.

But for McMillin, the company's growth has never been solely about building a larger practice.

It has been about building a different kind of healthcare company.

“I didn't start Custom Care Rehab because I wanted to build a big physical therapy company. I started it because I believed there had to be a better way to deliver care—for patients and for the clinicians providing it.”

It also shaped the company's approach to its employees.

Rather than recreating the volume-driven model McMillin had experienced in traditional healthcare settings, Custom Care Rehab was intentionally designed around clinician autonomy, flexible scheduling, professional growth, and a shared commitment to treating patients with dignity.

Today, the Custom Care Rehab team includes physical therapists with expertise spanning orthopedic rehabilitation, vestibular therapy, concussion care, women's health, neurological rehabilitation, and other specialized areas of care.

The company's team-centered approach was a central part of McMillin's CO—100 application, which described the goal of creating an ecosystem where physical therapists have greater autonomy, flexibility, and opportunities for financial growth.

The CO—100 recognition comes as Custom Care Rehab continues to expand its concierge healthcare model throughout Northeast Ohio.

For McMillin, however, the recognition represents something larger than business growth.

It represents proof that healthcare can be built differently.

The U.S. Chamber of Commerce will bring the 2026 CO—100 honorees together in Washington, D.C., October 19–21 for the CO—100 VIP Experience and related events, including the CO—100 Small Business Forum.
