Medicare will typically cover outpatient physical therapy if all of the following apply:
There is a documented injury or impairment.
You must have a clear medical diagnosis or physical limitation affecting your ability to perform daily activities like walking, bathing, getting dressed, or cooking.Treatment is goal-directed.
Therapy must have measurable goals (like improving range of motion, reducing fall risk, or returning to walking independently) that show progress over time.There is potential for improvement.
Medicare requires that your condition can reasonably be expected to improve or be maintained with skilled intervention.Documentation supports the need.
Your physical therapist must regularly document your progress and justify continued care.
🚫 When Medicare Does NOT Cover Therapy
Medicare does not cover physical therapy simply because it “feels good” or seems helpful. Some examples of non-covered situations include:
General stretching or massage for mild discomfort

Exercises to improve athletic performance or posture without a functional limitation

Physical therapy services while receiving home health care from a separate agency
It’s also important to note that Medicare has visit thresholds (often around 10–25 visits per episode), and continued care beyond that requires additional justification. After a certain point, your therapist may recommend discharge or transition to a self-pay wellness or maintenance program.
