As 2025 comes to a close, we look back with gratitude, pride, and excitement for the future. What began as a one-woman mission to bring exceptional, personalized physical therapy directly into the homes of Northeast Ohio families has grown into a thriving, multi-specialty practice serving communities across the region.
Founder of Custom Care Rehab, Amy McMillin, started her career in physical therapy in 2007 in the Northeast Ohio area. She graduated from Akron University with a Bachelor of Education and earned her Master of Science in Physical Therapy from Walsh University. After years of experience in orthopedic and geriatric physical therapy, Amy honed her specialty in inner ear disorders, migraines, and concussions.
In 2016, she earned her Herdman Vestibular Certification from Emory University and later became an Impact Trained Physical Therapist (ITPT) for concussion management. Most recently, she completed the prestigious Johns Hopkins University Vestibular Competency course in 2023. Her passion for helping people move, live, and feel better at home remains the heartbeat of Custom Care Rehab.
What started with one therapist and just 11 visits in the first month of business has grown into a team of five full-time physical therapists completing over 3,700 visits this year.

Meet Our Physical Therapists
Meghan Fonce, PT, DPT
Orthopedic & Manual Therapy Specialist | Strength Training for Runners | Balance & Postural Training
Meghan at work, helping with chronic neck pain.
Meghan earned her Doctorate in Physical Therapy from the University of Mount Union and brings a strong clinical background in orthopedic rehabilitation, balance training, and postural correction. She specializes in cupping, myofascial release, deep trigger point work, and kinesiotaping — techniques she blends with customized exercise programs for lasting results.
Passionate about working with runners, weekend warriors, and young female athletes, Meghan draws on her coaching background to build strength, confidence, and injury resilience. She lives in Medina with her husband, son, and their chickens, and brings a warm, energetic presence to every visit.

Maddie Lautt, PT, DPT
Orthopedic Physical Therapy | Vestibular Rehab | TMJ Treatment | Certified Dry Needling Clinician
Maddie earned her Doctorate in Physical Therapy from the University of North Dakota and joined the Custom Care Rehab team in February 2025. With three years of experience, she brings highly specialized skills in dry needling, cupping, deep tissue work, and instrument-assisted soft tissue mobilization.
She enjoys guiding clients of all ages toward pain-free movement and an active lifestyle. Maddie is a yoga sculpt instructor, pickleball player, reader, photographer, and unapologetic coffee enthusiast. She and her husband, Wyatt, recently relocated with their two cats from Fargo to Cleveland, where she is excited to build long-lasting one-on-one connections with clients.

Robbie Whittaker, PT, DPT
Orthopedics | Chronic Pain | Vestibular Rehab | Dry Needling | Support for Veterans & First Responders
Robbie earned her Doctorate in Physical Therapy from Belmont University in Nashville. She has extensive experience treating orthopedic injuries, chronic pain, and vestibular conditions using a blend of exercise, manual therapy, and dry needling.
Fun Fact: Robbie was pregnant with twins during graduate school!
Coming from a large military family and married to an EMT and veteran, Robbie carries a heartfelt passion for helping veterans, active-duty military members, and first responders. She is known for her ability to connect deeply with people, motivate them, and guide them toward achievements they didn’t think were possible.

Chris Smolko, PT
Orthopedic Specialist | Spine Care | Joint Manipulation | Athlete Performance
Chris brings over 20 years of experience after earning his Bachelor of Science in Physical Therapy from Youngstown State University. With advanced training in spinal mobilization, joint manipulation, and dry needling, he is highly skilled in treating low back pain, knee injuries, neck pain, shoulder injuries, and complex orthopedic conditions.
Known for his calm demeanor and deep empathy, Chris builds trust quickly and becomes the long-term therapist patients request again and again. He enjoys helping athletes of all ages stay active, enhance performance, and return to the activities they love.

Erin Porter, PT, DPT, CNS
Neurological Rehabilitation | Stroke Recovery | Spinal Cord Injury | Dry Needling
Erin is a dedicated therapist and mother. We are excited to have her as a part-time PT with CCR
Erin earned her Doctorate of Physical Therapy from the University of Mount St. Joseph in 2012 and brings more than 13 years of experience in inpatient rehab and outpatient neuro settings. She specializes in spinal cord injury recovery, stroke rehabilitation, and complex neurological conditions.
Certified in dry needling in 2022 and achieving her Certified Neuro Specialist (CNS) credential in 2024, Erin is dedicated to restoring mobility, independence, and confidence in every patient she serves. She lives in Northeast Ohio with her family and two dogs and approaches care with compassion and clinical excellence.
