From Solo Physical Therapist to One of America's Top 100 Small Businesses: The Custom Care Rehab Story

Custom Care Rehab named a 2026 CO—100 Culture Champion by the U.S. Chamber of Commerce

In 2020, Custom Care Rehab was not a team.

It wasn't a large healthcare company. It wasn't a recognizable brand. It was one physical therapist with an idea.

Today, Custom Care Rehab has been named one of America's Top 100 Small Businesses by the U.S. Chamber of Commerce as a 2026 CO—100 Culture Champion.

For founder and CEO Amy McMillin, PT, MSPT, the recognition is about much more than business growth.

It represents the opportunity to build healthcare differently.

Building Custom Care Rehab During COVID

Custom Care Rehab was founded during the COVID-19 pandemic after Amy McMillin saw firsthand how difficult it could be for patients to access the physical therapy they needed through a traditional clinic model.

As a physical therapist with more than 15 years of clinical experience, Amy had spent years working within traditional healthcare environments. She understood the value of excellent clinical care—but she also saw an opportunity to create something more personal, flexible, and accessible.

So she started Custom Care Rehab as a solo physical therapist.

The vision was simple:

Bring expert physical therapy directly to the patient.

Instead of requiring patients to rearrange their lives around a clinic appointment, Custom Care Rehab would bring one-on-one physical therapy into the environments where people actually live and work.

That vision became the foundation for a concierge physical therapy model serving clients throughout Cuyahoga, Summit, and Stark counties in Northeast Ohio.

The U.S. Chamber of Commerce now recognizes Custom Care Rehab as a 2026 CO—100 honoree in the Culture Champions category.

From One Physical Therapist to a Growing Northeast Ohio Team

What began as a one-person practice has grown into a team of physical therapists and wellness professionals serving clients throughout Northeast Ohio.

Today, the Custom Care Rehab team brings together clinicians with expertise across a variety of areas, including orthopedic rehabilitation, vestibular therapy, neurological rehabilitation, women's health, concussion care, and wellness-focused services.

But building a team was never simply about growing the number of therapists at Custom Care Rehab.

It was about creating a workplace where great clinicians could practice differently.

From the beginning, Amy wanted to build a company that valued clinical autonomy, flexibility, professional growth, and meaningful relationships with patients.

That philosophy became an important part of the company's culture—and ultimately became the reason Custom Care Rehab was recognized as a Culture Champion by the U.S. Chamber of Commerce.

Why Culture Matters in Healthcare

Healthcare is often measured by numbers.

How many patients were seen?

How many visits were completed?

How quickly can the schedule be filled?

But Amy believes there is another metric that matters just as much:

How do the people providing the care feel about the work they are doing?

Custom Care Rehab was intentionally built to give physical therapists more autonomy over how they practice, greater flexibility in their schedules, and the opportunity to develop professionally while providing highly individualized care.

The goal is not simply to create a place where therapists work.

It is to create a place where clinicians can thrive.

That philosophy was central to Custom Care Rehab's CO—100 application, which focused on moving away from a volume-driven healthcare model and toward a high-touch approach that prioritizes human dignity, clinician autonomy, and patient-centered care.

The U.S. Chamber's Culture Champions category recognizes businesses that have cultivated rewarding company cultures, attracted and retained dedicated employees, and built successful organizations around shared purpose.

For Custom Care Rehab, that recognition is especially meaningful because the company's culture was never an afterthought.

It was part of the plan from the beginning.

Concierge Physical Therapy Designed Around the Patient

The culture inside a healthcare company ultimately affects the experience patients receive.

At Custom Care Rehab, that means physical therapy is designed around the individual rather than around a traditional clinic schedule.

Our therapists provide personalized, one-on-one care in clients' homes, workplaces, and other private settings throughout Northeast Ohio.

For busy professionals, executives, active families, and individuals who value privacy and convenience, concierge physical therapy can offer a different experience from the traditional clinic model.

There is no crowded waiting room.

There is no rushing from one patient to another.

And there is no expectation that every person should fit into the same treatment schedule.

Instead, the focus is on the individual in front of us.

Our goal is to help patients move better, feel stronger, recover from injury, and maintain the physical health necessary to continue doing the things they enjoy.

Serving Cuyahoga, Summit, and Stark Counties

Custom Care Rehab proudly serves clients throughout Northeast Ohio, including communities across Cuyahoga, Summit, and Stark counties.

Our mobile model allows our therapists to bring personalized physical therapy directly to the communities we serve.

Whether someone is recovering from an orthopedic injury, dealing with a neurological condition, returning to activity after an injury, navigating balance or vestibular challenges, or simply looking for a more individualized approach to maintaining their health and mobility, our team is committed to meeting patients where they are.

Healthcare should work around your life—not require your life to revolve around healthcare.

That belief remains at the heart of Custom Care Rehab.

What Being Named a CO—100 Culture Champion Means to Us

The U.S. Chamber of Commerce's CO—100 is its annual recognition of 100 outstanding small and mid-sized businesses across the United States.

The 2026 honorees were selected from more than 12,000 applicants and represent businesses across 36 states and 15 industries. The honorees were recognized for qualities including innovation, growth, resilience, community impact, customer commitment, and company culture.

Custom Care Rehab was honored specifically as a Culture Champion.

That distinction means a great deal to us.

Because building a successful business is important.

But building a successful business without losing sight of the people who make it possible is even more important.

Our patients trust us with their health.

Our therapists trust us with their careers.

And we believe both deserve a healthcare company built around relationships, respect, autonomy, and a genuine commitment to doing things differently.

A Different Definition of Success

When Amy started Custom Care Rehab, the goal wasn't simply to build a bigger physical therapy practice.

It was to create something better.

A different kind of healthcare company.

A company where patients could receive exceptional care in the comfort and privacy of their own environment.

A company where physical therapists could have more autonomy and flexibility.

A company where relationships mattered.

And a company where growth did not have to come at the expense of the human side of healthcare.

As Amy wrote in her CO—100 application:

“Success isn't about accumulation; it is about the freedom to make an impact.”

That idea continues to guide Custom Care Rehab today.

Being named one of America's Top 100 Small Businesses is an incredible honor, but it also feels like a reminder of why Custom Care Rehab was created in the first place.

There is room to build healthcare differently.

And we are grateful to continue building that future here in Northeast Ohio.

Looking Ahead

Custom Care Rehab will join the other 2026 CO—100 honorees in Washington, D.C., October 19–21 for the CO—100 VIP Experience and related events celebrating America's leading small businesses. The U.S. Chamber notes that the 2026 honorees collectively represent more than $340 million in annual revenue and more than 2,500 employees across 36 states.

For our team, this recognition is an opportunity to celebrate how far we've come—and to look forward to where we can go next.

From one physical therapist working to create a different model during COVID to a growing team serving communities throughout Northeast Ohio, Custom Care Rehab's story is still being written.

And we are just getting started.

Thank you to our patients, referral partners, community, and team for being part of the Custom Care Rehab story.
